Remote Licensed Practical Nurse (LPN) - Full-time/Part-time Opportunities

As a Licensed Practical Nurse, you will play a vital role in our Medication Management and Care Coordination department, conducting telephonic patient outreach and interviews to ensure adherence and patient safety.

About the Role

• Perform telephonic patient interviews in English and Spanish to discuss medications, adherence, and health status
• Provide therapeutic recommendations and accurately document clinical data in patient charts
• Identify potential issues and maintain current knowledge of medications, HIPAA, and CSS Health policies and procedures
• Collaborate with our team of pharmacists, clinicians, and support staff to deliver exceptional patient care

Requirements

• Graduation from an accredited nursing program (LPN or LVN) with an active license in good standing
• At least 1 year of experience in the healthcare industry or call center experience preferred
• Intermediate understanding of Microsoft Office Suite and database navigation
• Strong multitasking abilities, attention to detail, and excellent communication skills
