We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Heritage Valley LTACH. As an LPN, you will play a vital role in providing high-quality patient care and supporting our mission to deliver exceptional healthcare services.

Job Summary:

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse (RN). This includes administering medications, performing therapeutic procedures, and documenting patient information in a timely and accurate manner. The ideal candidate will have excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient care.

Responsibilities:

• Provide direct patient care, including administering medications and performing therapeutic procedures
• Document patient information accurately and in a timely manner
• Collaborate with the interdisciplinary team to develop and implement patient care plans
• Monitor patients for changes in medical condition and report concerns to the RN and physician
• Provide education and support to patients and their families

Qualifications:

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania
• Current BLS certification
• One year of inpatient medical-surgical nursing experience preferred
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment
