About the position The Registered Nurse - RN - Case Management position at Talvora Health is a full-time, remote role focused on engaging and advocating for vulnerable members with complex medical, behavioral, and social challenges. Responsibilities
Assess, plan, implement, and coordinate all case management activities with members. , Evaluate the medical needs of members and facilitate their overall wellness. , Develop proactive courses of action to enhance short and long-term outcomes for members. , Conduct evaluations of members' needs using clinical tools and data review. , Incorporate strategies to reduce risk factors and address complex health and social indicators. , Conduct assessments considering various sources of information for co-morbid and multiple diagnoses. , Use a holistic approach to assess the need for referrals to clinical resources and interdisciplinary team members. , Collaborate with supervisors and key stakeholders to overcome barriers in meeting healthcare goals. , Utilize case management processes in compliance with regulatory and company policies. , Employ motivational interviewing skills to engage members and assess their health needs.
Requirements
Registered Nurse with an unrestricted nursing license and multi-state/compact privileges. , 3+ years of clinical/nursing practice experience. , 1+ year(s) of medical case management experience, care coordination, or discharge planning. , 3+ years of experience with personal computers, keyboarding, multi-systems navigation, and MS Office Suite applications. , COVID vaccination is required.
