Remote Hospice Social Worker

Key Responsibilities
- Conduct assessments to determine the social, emotional, and financial needs of patients and their families.
- Develop and implement care plans that address the unique challenges faced by patients in hospice care.
- Offer emotional support and counseling to patients and families coping with end-of-life issues.
- Collaborate with healthcare teams to ensure coordinated and comprehensive care.
- Educate families about available resources and services, and assist them in navigating the healthcare system.
Required Qualifications
- A Master's degree in Social Work (MSW) from an accredited institution.
- Current state licensure as a clinical social worker.
- Experience in hospice or palliative care settings is preferred.
- Strong communication and interpersonal skills, with a genuine compassion for patients and their families.
- Ability to work independently in a remote setting while staying connected with the team.
