New York Oncology Nurse Navigator
To support a compassionate cancer care experience, the full-time Oncology Nurse Navigator will provide triage, support, and education to members throughout their cancer journey, working remotely and engaging in clinical assessments and care coordination. Key responsibilities Conduct comprehensive clinical assessments and manage member health, addressing clinical issues and facilitating end-of-life care discussions Establish trusting relationships with members and their care network, ensuring adherence to care protocols and documentation standards Collaborate with healthcare teams to monitor member progress and provide targeted support throughout the oncology care continuum Required qualifications Active, unrestricted compact Registered Nurse (RN) license; degree in Nursing (ADN or BSN) required, BSN preferred At least 5 years of nursing experience, with a minimum of 3 years in solid tumor oncology nursing Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM required or to be obtained within 2 years of hire Compact and single-state licensure in New York and/or Illinois preferred Fluency in Spanish, both written and verbal, is preferred Apply tot his job Apply To this Job
