Key Responsibilities

  • Act as the attending physician for residents in skilled nursing facilities, ensuring they receive comprehensive primary care tailored to their needs.
  • Collaborate effectively with Nurse Practitioners (NPs) and Physician Assistants (PAs) to enhance patient care, depending on the volume and acuity of patients at each facility.
  • Provide high-quality diagnostic and therapeutic care for various acute and chronic conditions, such as infections, hypertension, and diabetes, with an emphasis on patient-centered care.
  • Utilize Theoria's proprietary electronic health record (EHR) system, ChartEasy™, for efficient patient documentation and care coordination.
  • Engage actively in initiatives such as Chronic Care Management and Transitional Care that aim to improve patient outcomes and quality of care.
  • Supervise and guide NPs and PAs, ensuring adherence to quality standards and clinical protocols.
  • Leverage online medical tools and state systems for administrative functions, including death certificates and other essential documentation.

Patient Population / Client Focus As a Physician with Theoria Medical, you will work with diverse patient populations in skilled nursing facilities, addressing both complex and routine medical needs. Your expertise will play a vital role in improving the health outcomes of residents, many of whom may have chronic diseases requiring careful management and ongoing support.

Qualifications

  • A bachelor's degree in a relevant field is required; a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) is essential for practice.
  • Valid medical licensure in North Carolina, with reimbursement for licensure expenses provided.
  • Strong communication skills, ability to build rapport with patients, families, and interdisciplinary teams.
  • Proficient with electronic health record (EHR) systems, particularly ChartEasy™, PointClickCare, MatrixCare, and American HealthTech.

Preferred Experience

  • Previous experience in post-acute care or primary care settings is highly desirable.
  • Experience working with chronic disease management and remote patient monitoring.
  • Familiarity with the challenges and opportunities in skilled nursing facilities.
